Dosimetry Badge Terms and Condition

Dosimetry Badge is a service provider of radiation detection and measurement products. The company provides dosimetry services on a subscription basis. Subscribers sign up for services either on the website or through our live customer service specialists. Subscribers will then receive a shipment of personalized dosimeters for their employees for each wear period. Wear periods may run monthly or quarterly as selected by the subscriber. Subscribing to our service constitutes agreement with the following terms of service.
Badges are shipped within 3-business days of a new order and are sent standard parcel post. New wear period badges are then sent out 10-14 days prior to the expiration of currently worn badges to avoid interruption of service. Subscriptions are automatically renewed each period unless notified by subscriber to cancel service. By signing up for this services, subscriber agrees that service is automatically renewed 45 days in advance of next scheduled wear date for successive like periods of service. Continued use of the subscription service constitutes acceptance of these terms. By signing up for service, you are agreeing to allow Dosimetry Badge to charge your credit card to automatically process payments for subsequent periods. This process can only be stopped by providing 45 days written notice.
Payment of accounts are prepaid and made quarterly. This means that subscriptions prepay for the first 3 months of service upon sign up. With services provided in an uninterrupted manner (by law), the terms of service for prepaid accounts require that a positive cash balance be maintained to ensure continuous dosimetry service. Invoices are issues quarterly with the first invoice being issues 45 days after account initiation. Payment terms on these invoices are NET 30 days. Subscribers may select automatic payment via credit or mail a check. The mailing address for payment is:

Billing is based upon actual customer usage (defined as the moment the manufacturer begins personalizing dosimetry device) and the Dosimetry Badges price list is in effect at the time of shipment. Ancillary charges (such as rush processing, expedited shipments or unreturned dosimeter fees) are in addition to subscription fees.
Shipments are not made to accounts with unpaid balances greater than 60 days or to prepaid accounts with zero or negative cash balances.
Cancellation requires 45 days written notice. Badges are prepared by the manufacturer for individual wearers up to 45 days prior to their start date. Thus accounts will be billed should written notice not be received prior to 45 days. Our services are subscription based. If you cancel your subscription, your on-line services will remain active for the remainder of the time period for which you paid. DOSIMETRY BADGES DOES NOT REFUND THE REMAINING UNUSED PORTION OF YOUR SUBSCRIPTION. Certain situations such as loss of property due to building damage beyond subscribers control, credit card fraud and/or acts of God may be evaluated on a case-by-case basis as they pertain to refunds of prepaid services. Badges may be prepared up to 30 days prior to wear date. Cancellation notices should be emailed to info(at)dosimetrybadge(dot)com.
Dosimetry Badges pricing is a tiered-based pricing determined by the number of active badges for each customer. Pricing is locked in during the new subscriber period. This pricing will remain in effect for a minimum of 12 months but is then subject to change due to material costs. Due to the fluctuating cost of raw materials, Dosimetry Badges may raise or lower the prices on badges one time per year. Subscribers will be notified of any changes in price at least 45 days prior to the price change taking effect.
Dosimetry Badges dosimeters contain valuable components and remain the property of the manufacturer. These badges must be returned at the end of the wear cycle. Unreturned or lost badges are subject to the lower of the current, published replacement prices 60 days after the wear period or $20 per unreturned badge. By signing up for service, you are agreeing to allow Dosimetry Badge to charge your credit card for any and all unreturned badges at the assessed price of $20 per unreturned badge (with an unreturned badge being defined as 60 days after the wear period has ended). It is the responsibility of the subscriber to return all badges affiliated with the account including the control badge. Badges returned after the 60 day period are entitled to a $10 per badge refund – conditioned upon the payment of the lost badge fee.
Dosimetry Badges does accept purchase orders. Purchase orders are for subscriber tracking purposes and in no way obligate Dosimetry Badges to any terms or pricing. Purchase Orders should be emailed to info(at)dosimetrybadge(dot)com.
Dosimetry Badge offers a money back guarantee on our new accounts. If, during the first 30 days of service, you are not 100% satisfied with the service that we provide, simply email a cancellation request and request for a refund to info(at)dosimetrybadge(dot)com. Your 30 days begins upon delivery of your first set of badges for all new accounts. Written request must be received with 30 days otherwise, the refund offer is no longer valid. Refunds will be issued once all outstanding badges have been returned to our NVLAP approved laboratory.
